What information needs to be entered into an EC sales list?

My client has made sales to a customer in France for the first time this quarter. I've requested an EC sales list from HMRC, but I'm not sure what information needs to be entered on to the form?
Can anyone please advise?

Although it looks a bit daunting, it's fairly simple to complete. You need to ensure you have the customers VAT registration number (this may be in a slightly different format to a UK number) and the amount of the sale.
